It is offered in face-to-face groups across the country (Cancer Transitions dates and locations) and also online. The Cancer Transitions: Moving Beyond Treatment workbook, and the Facing Forward: Life After Cancer Treatment booklet from the NCI to guides discussion throughout the program, along with other interactive content. Individual Grant Program

by the United Breast Cancer Foundation

http://ubcf.org/individual-grant/

The United Breast Cancer Foundation's (UBCF) mission is to make a positive difference in the lives of those affected by breast cancer. We aid in providing screening, treatment, after-care, educational material, patient and family assistance and information.

Created in 2007 with the intention to financially assist women and men who are managing breast cancer, this program offers customized support geared towards each individual’s personal needs and circumstances. Like our clients, no two grants are ever alike. We understand that breast cancer has a far-reaching affect on every member of the family; therefore, when crafting an Individual Grant, UBCF considers not only the breast cancer patient’s needs, but the needs of the entire family.

After careful and thorough assessment of the client’s needs, UBCF constructs a grant that will best serve the needs of the client, both in the short term and long term. We work closely with each client to find opportunities to lessen or resolve financial burdens. We have found over the years that when our client’s experience a reduction in stress, they are more focused on their healing journey, more capable to manage the challenges of overcoming breast cancer.

Currently, the Individual Grant Program provides financial support for the following types of expenses: medical procedures, prescription medications, COBRA insurance coverage, housing expenses, utilities, prosthetic devices, therapeutic treatments, transportation expenses, vehicle insurance, healthy and nutritious foods, house keeping services and holiday gifts.

Please be advised that the application and review process can take up to 12 weeks. To make the process as smooth as possible please supply all the necessary documentation in your application. All applicants will receive a grant, but have patience in our process!

A breast cancer diagnosis can destroy the financial health and well-being of the patient and their family. While in active treatment, many patients are unable to work and experience a loss of income that can result in catastrophic financial losses and the need to rebuild financial health. The Pink Fund provides financial support to help meet basic needs, decrease stress levels and allow breast cancer patients to focus on healing while improving survivorship outcomes.

The Pink Fund distributes short-term financial aid, for basic living expenses, on behalf of breast cancer patients who have lost all or a part of their income during active treatment. Payments are made directly to the patient’s creditors.

Short term is defined as a period of up to three consecutive months of non-medical financial assistance with a cap of $3,000, based on available funding. Please note our ability to provide support varies based on the number of qualified applicants and available funding. We suggest you seek out additional resources. Please see our Resources listed to the left.

Basic living expenses considered are health insurance premiums, mortgage or rent payments, car payments, car insurance, utilities and car repairs. The Pink Fund does not make payments for any medical treatments, prescription drugs (including hormone therapy), medical co-pays, insurance deductibles, prostheses, wigs, food, gasoline.

Active treatment is defined as the period after a positive diagnosis of breast cancer has been made (with a diagnostic biopsy), and during which therapies are being administered, including surgical procedures (e.g., single or bi-lateral mastectomy, lumpectomy, axillary dissection or sentinel node biopsy), chemotherapy or radiation. For the purposes of The Pink Fund, active treatment does not include long-term hormonal therapies.

Recovery is defined as a time period determined by your physician and includes a return-to-work date.

Should an applicant qualify, The Pink Fund will make direct payments to the recipient’s creditors. No funds are directed to the recipients themselves. Expenses withdrawn automatically from a bank account will not be considered unless The Pink Fund can make the payment directly to the creditor.

Who does NOT qualify for aid from The Pink Fund?

You do NOT qualify if you were unemployed at the time of your diagnosis, or cannot show a reduction in your income as a result of your diagnosis.
You do NOT qualify if you are currently receiving monthly financial aid through the State and/or Social Security Disability Insurance Benefits (SSDI).
You do NOT qualify if you are retired and receiving a monthly pension check from your former employer and do not require a second job to pay for basic monthly living expenses.
You do NOT qualify if you are working in an unrecognized revenue producing position in which you are currently not paying federal or state taxes, or receiving cash under the table which is not reported to the government

HOTEL LODGING by the American Cancer Society

Call 800-227-2345 to get services.

The American Cancer Society’s mission is to save lives, celebrate lives, and lead the fight for a world without cancer.

The American Cancer Society, in a cooperative effort with hotels across the country, provides overnight accommodations to cancer patients who must travel for outpatient treatment and need assistance with lodging. Getting the best care sometimes means cancer patients must travel away from home. This can place an extra emotional and financial burden on patients and caregivers during an already challenging time. We are trying to make this difficult situation easier for both cancer patients and their families through our lodging programs. All accommodations are provided based on eligibility requirements and are subject to availability and to restrictions imposed by the participating hotels. Requests for lodging are met on a first-come, first-served basis.

Since 1980, the Make-A-Wish Foundation has enriched the lives of children with life-threatening medical conditions through its wish-granting work. The Foundation's mission reflects the life-changing impact that a Make-A-Wish experience has on children, families, referral sources, donors, sponsors and entire communities.

Although it has become one of the world's most well-known charities, the Make-A-Wish Foundation has maintained the grassroots fulfillment of its mission.

Mercy Medical Angels (MMA) - Ground Transportation Program

  888-675-1405

http://mercymedical.org/ground-transportation/request-assistance/

Description

Mercy Medical Angels (MMA) ensures that no needy patient is denied access to distant specialized medical evaluation, diagnosis or treatment for lack of a means of long-distance medically-related transportation; further to ensure the provision of urgent transportation in situations of compelling human need and homeland security (disaster response) emergencies. MMA bridges the access gap by providing charitable medical transportation throughout the United States by using one or more of the modes of transportation. Commercial Airline Tickets, Volunteer Pilot Operations and Long Distance Ground Transportation.

MMA’s Angel Wheels program ensures travel for low-income patients by providing gas cards and commercial bus or Amtrak tickets. Patients (with or without escort) must be ambulatory and require no medical monitoring or care en route. These trips are for non-local medical care or for patients returning home after a distant appointment or hospitalization. The typical trip does not exceed 300 miles.
