Hospice Care in Nursing Facilities​

Hospice Care in Nursing Facilities program provides care and treatment necessary for physical, emotional and spiritual well being, wherever you call home including a skilled nursing facility, continuing care retirement community, rest home or assisted living facility.

Hospice Care at Home-Kids Path Grief Counseling

Kids Path Grief Counseling program helps meet the special needs of children and teens who have experienced the death of a loved one. Grief counseling includes short term individual, family, and group counseling for children and adolescents under the age of 18.

Kids Path - Medical

Kids Path Medical program provides the special needs of children and teens who are diagnosed with any life limiting illness. Hospice care is offered for children ages birth to 18 years with a life threatening illness when the childs life expectancy is measured by months rather than years.

Care Connection

Care Connection program helps on coordinating care, and reducing the pain and other distressing symptoms of a serious life limiting illness. Palliative care consultations help improve the quality of life for patients receiving curative treatment at any stage of a serious illness.

Hospice Care at Home

For patients and families dealing with a serious illness, Hospice of the Piedmont team members visit the home to provide comfort and support. Expert medical care and support services are available for patients of all ages with any life-limiting illness.

Hospice Care at Home-The Bereavement Counselor

The Bereavement Counselor program provides specialized grief support to families following the loss of a loved one. Support groups, reading materials and individual counseling can continue for up to a year.

Hospice Home at High Point

Hospice Home at High Point program provides its exceptional end of life care in a comfortable, home like setting. The facility is designed to meet the needs of terminally ill patients, as well as those of visiting family members and friends. The goal of Hospice care is to make the patient feel more comfortable, more at peace and more in control.

Veterans have protected and cared for us, and our country. Through Hospice for Heroes, we honor Veterans by providing a hospice care program that recognizes and responds to the unique needs of the Veteran and his/her family.

Hospice for Heroes integrates all the holistic and specialized pieces of Hospice Care, like pain and symptom management, with special services designed just for veterans. All Veterans in the Hospice for Heroes service get pinned by a ranking member of the military force in which he or she served; visits from volunteers who are also Veterans; and guidance with military benefits, records recovery and burial arrangements.

Our Hospice for Heroes Veterans Program enables a Veteran, and his/her family to make informed decisions, and participate in care planning when facing a life limiting illness. By gaining knowledge and expert support from our team, a Veteran and his/her family are better positioned to live each day to its fullest.

The components of the Hospice for Heroes Veterans Program include but are not limited to:
-A focused admissions process, including a specific Veteran admission information packet
-A team who are not only experts in pain and symptom management but also are experts in identifying and meeting the unique needs of Veterans and their families, insurance coverage of care, and available Veteran benefits
-An individualized care plan which is Veteran and family centered and includes the Veteran’s military experiences

We actually make house calls and provide primary medical care for chronic and acute illnesses. Our service is the perfect alternative for patients who have difficulty traveling to a doctor's office. Patients receive quality healthcare in the comfort of their own homes. Everything from routine checkups to lab work, x-rays, EKGs and ultrasounds are all provided in-home. Our unique program helps patients live independently longer, with less emergency room visits, less hospitalizations and helps delay the need for skilled nursing home care. We are trained to work closely with the patient, their families, caregivers, home health nurses, therapists and social workers. Upon request, MD2U will also work with the patient's existing primary care physician.

Family Lives On Foundation

Family Lives On Foundation is a 501(c)(3) nonprofit organization that serves grieving children, teens and families nationally. We support children and teens whose mother, or father, has died. The Tradition Program enables them to celebrate traditions or continue activities they used to do with their mom or dad. Subject to program guidelines, Family Lives On arranges and pays for the entire tradition request for each child annually until the child turns 18. Only the request children and family members participate in the tradition.

The criteria for children/teens to be eligible for Family Lives On's free services include:

-They are between the ages of 3 and 18. 
-Their mothers or fathers have died.
-They live in the United States of America.
-Their families must directly request Family Lives On's support. 

After Family Lives On receives an enrollment form from a family, two trained volunteers conduct a Skype "Family Meeting" with the children and surviving parent or guardian to discuss traditions that the child shared with his or her deceased parent. This process not only conveys to the family the value of remembering, but also helps the child/teen to choose a tradition that he wants to continue.

Funeral or Cremation Assistance

Skyler’s Gift Foundation started by Tiffany and Stephen Bowen to provide financial support to families in need that have lost their newborn infants from complications of premature birth. They believe no one should have to face this loss alone and especially without the needed resources in place

For those born at 24 weeks like Tiffany and Stephen Bowens’s twin boys, Stephen and Skyler, there is only a 39% chance of survival. Most infants born prior to 24 weeks have little to no chance of survival even with modern medical technologies. While many services are in place for families during hospitals stays in the NICU, after a loss the support is no longer there. That’s where Skyler’s Gift steps in. Skyler’s Gift raises funds to support families in need and also supports ongoing research to prevent premature births and deaths and raise awareness of this very personal issue.

Skyler’s Gift’s primary goal is to help as many families as possible in this most difficult time of their life. In order to do this and maximize how many families receive assistance we rely on accurate financial information from applicants. Completion of our application, available on our website, does not guarantee financial assistance. You will be notified within 48 hours if approved for assistance. Skyler’s Gift Foundation will help with the cost of funeral and burial or crematory cost up to $1000. 

-Skyler’s Gift does not send payment directly to any applicants. All payments go to the funeral home and/or cemetery.
-Skyler’s Gift does not review incomplete applications. All required information and material including proof of finances, the funeral home and/or cemetery quotation, and proof of residence must be submitted before we can provide a decision.
